Which FMCSA forms are needed?
Every supported request generates an MCS-150 to update the USDOT record. If the carrier also has MC, MX, or FF operating authority, the request also generates MCSA-5889.
What documents does FMCSA require?
Every business name-change request requires the front and back of the authorized officer’s driver’s license and Articles of Incorporation, amendments, or another applicable state-issued name-change record. MX carriers may also need a Tarjeta de Circulación.
Does a name change transfer operating authority?
No. A simple name change does not transfer authority to a new owner. Changes in ownership, management, or control must be screened to determine whether an authority transfer or new registration is required.
What does the service cost?
